Health Professional's Report (WSIB Form 8)
In Ontario, the Health Professional's Report is WSIB Form 8 — the first medical report after a work injury or occupational disease. Other provinces use different names (WorkSafeBC 8/11, CNESST attestation). If you are holding Form 8, you are reporting to WSIB, not to those boards.
What Form 8 is for
Complete Form 8 when you first see a work injury that may need more than first aid. WSIB uses it with the worker's Form 6 and the employer's Form 7. Your piece is diagnosis, cause as told to you, and return-to-work status.
Download the current Form 8 from WSIB. Progress is a different report (Form 26 or the current equivalent). Do not keep filing Form 8 as a progress note.
Occupational disease is still Form 8
Noise, dermatitis, and gradual-onset back pain still need a first report. The 'date of injury' may be date of first awareness. Say that. Do not leave the date blank because there was no single fall.
Who completes it
- Physicians and nurse practitioners, and other health professionals WSIB lists for Form 8.
- The clinician who did the assessment. A student note with your signature is your report.
- Chiropractic and physiotherapy have their own WSIB reporting in some programs. Do not use Form 8 for those if WSIB gives them another number.
How to complete Form 8
- Worker's name, DOB, and employer as given. Claim number if known.
- Date of accident or awareness, and date of this visit.
- Worker's description of what happened, in their words, one or two sentences.
- Objective findings and diagnosis. 'Low back pain' plus a straight-leg raise and a work status is better than a paragraph of sympathy.
- Is it compatible with the described work incident? If you are unsure, say unsure.
- Return to work: regular, modified with listed limits, or off. Duration of modified work.
- Treatment and next visit. Imaging only if you ordered it.
WSIB bills using the current fee schedule for Form 8. That is separate from the clinical content. This page is not a billing manual.
Worked example (fictional)
PSW — first visit for lumbar strain
Fictional 33-year-old. Do not copy onto a real Form 8.
DOI 26 Aug 2026. Seen 27 Aug 2026, family practice. Claim number unknown. Incident (worker's words): 'Boosting a resident up the bed with a colleague; felt a pop in my low back on the left.' Exam: gait independent, flexion to mid-shin, SLR negative, no saddle, no weakness, no fever. No red flags. Diagnosis: lumbar strain, compatible with the described lift. No imaging. RTW: modified — no resident boosting, no floor lifts, 5 kg limit, 4-hour shifts × 10 days, then reassess. Regular duties not expected this week. Plan: stay active, NSAID, PT if not moving at day 10. Next visit 8 Sep. I am the FP. This is Form 8, not an employer sick note.
Why WSIB queries Form 8
- No work status.
- Mechanism missing, so it cannot be matched to Form 7.
- Occupational disease with no date of awareness.
- Filing Form 8 at week 6 as if it were progress.
- A diagnosis that cannot be reconciled with the exam (bilateral 'sciatica' with a normal neuro exam and no history of radiation).

Can an NP sign Form 8?
If WSIB's current Form 8 instructions include nurse practitioners, yes. Follow the professions on that PDF.
