WorkSafeBC Physician's Report (Form 8/11)
Form 8/11 is the physician's report to WorkSafeBC. Send it within three business days of the first visit if you think the worker may be off work beyond the day of injury. It is how the claim learns the diagnosis, the mechanism, and whether the worker can go back tomorrow.
What Form 8/11 is for
There is no separate contract required for a physician to treat a WorkSafeBC claim, but you do need to be in good standing with the College of Physicians and Surgeons of B.C., check claim status, and follow the Doctors of BC / WorkSafeBC services agreement and fee schedule.
Form 8 is the first report. Form 11 is progress. The combined 8/11 is what most clinics keep as a pad or PDF. Use WorkSafeBC's current file. Progress visits still need a work status, not a repeat of the original mechanism paragraph.
Claim status first
Verify the claim is accepted before you assume payment. WorkSafeBC has a claim-status tool for providers. If you saw the worker the day of injury and the claim is not yet in the system, still complete 8/11 with the facts you have.
Who completes it
- The physician who assessed the work injury.
- Nurse practitioners follow the NP reporting path WorkSafeBC publishes; do not assume 8/11 is interchangeable if the current guide says otherwise.
- Specialists have additional consult-report rules and fee codes. A specialist consult is not a substitute for the GP's first 8/11 if you were the first doctor.
How to complete the clinical fields
- Date of injury, date of this visit, and whether this is first report or progress.
- Worker's description of the incident in one or two sentences. Quote mechanism, not a legal conclusion about fault.
- Findings: the body part, the exam, and any imaging you have today. 'Tender' without a joint or a nerve is not enough.
- Diagnosis you are treating, and whether you think it is work-related. If you are unsure, say unsure and why.
- Work status with restrictions in job language: lift, carry, sit, stand, climb, use of the injured hand. Duration of those restrictions.
- Treatment and next visit. If you are sending to physiotherapy, say so. If you are not, do not leave 'rehab' blank as if someone else will invent it.
- Clinical impairment or recovery expectation only if the form asks and you can support it. Do not guess a percentage to be helpful.
Subsequent 8/11s should change. If week 4 looks identical to day 1, the claim will stall. Either function is moving or you need a different plan.
Worked example (fictional)
Carpenter — first visit after a saw injury
Fictional 41-year-old. Do not copy into a real Form 8/11.
First report. DOI 27 Aug 2026. Seen 27 Aug 2026, 16:40, urgent care. Incident: circular saw kicked back; laceration volar right index, dominant hand, through skin and possible FDS. Tetanus up to date. No other injury. Exam: 3 cm volar laceration crossing the PIP flexion crease. Sensation reduced on radial pulp. Cannot flex PIP independently. Radial pulse intact. No other hand injury. Impression: open injury, suspected FDS disruption, possible digital nerve. Work-related. Work: framing carpenter, both hands, saws and nails all shift. Off work. No use of right hand. Cannot climb or carry. Plan: irrigation, loose closure, splint, urgent hand-surgery referral tonight, antibiotics. Next physician visit after specialist. Not starting PT until the tendon plan is known. Claim status: not yet visible in the tool; worker says employer will file today.
Why 8/11 gets queried
- Sent late, after the worker has already been off a week with nothing on file.
- Work status 'off' with no restriction and no duration.
- Mechanism missing, so the claim cannot match the employer's report.
- Progress report that only says 'continue off work' with no exam.
- Wrong fee code or a report that does not match the invoice.

Frequently asked questions
When is Form 8/11 required?
WorkSafeBC asks physicians to send it within three business days of the first visit if the worker may be away beyond the day of injury. Check the current physician requirements page if your situation is a specialist follow-up or a very minor injury.
Is Form 8/11 the same as a physiotherapy report?
No. You report medical diagnosis and work status. Physiotherapy files a separate initial or discharge report. They should agree on body part and injury date.
Can I bill WorkSafeBC for completing 8/11?
Use the Doctors of BC / WorkSafeBC unique fee schedule and the current services agreement. This page is not a fee schedule.
