CNESST Attestation Médicale

The attestation médicale is the physician's statement to the CNESST after a lésion professionnelle — a work accident or occupational disease. It tells the Commission what you diagnosed, whether the worker can do the job, and when you expect consolidation. It is not a sick note for the employer written in French.

What the attestation is doing

Download the current form from the CNESST site (French for most physician documents). Médecins have a dedicated procédures et formulaires path. The worker also has their own claim steps. Your attestation is the medical spine of that file.

CNESST language is specific. Consolidation is the point where the injury is healed or stable, not 'the worker feels better.' Assignabilité is whether a temporary assignment is medically possible. Do not replace those ideas with English workplace jargon from another province's board.

French field names stay French

Write clinical facts in the language the form expects. Keep official terms: diagnostic, date de l'événement, consolidation, limitations fonctionnelles, assignation temporaire. Translating the headings on the PDF is how files get misread.

Who completes it

  • The treating physician (médecin qui a charge).
  • A designated health professional (professionnel de la santé désigné) when CNESST has appointed one for an opinion.
  • Other professions (physiotherapy, psychology) have their own CNESST forms. Do not use the physician attestation for those.

How to complete the clinical fields

  • Date de l'événement and the worker's description of the accident or exposure. One incident, not a career history, unless this is an occupational disease file.
  • Diagnostic you are treating. Name the structure. 'Douleur à l'épaule' is a symptom.
  • Exam and investigations available today.
  • Limitations fonctionnelles in task language: charge, hauteur, répétition, sitting, use of a limb. Duration.
  • Whether temporary assignment is possible, and what it cannot include.
  • Date de consolidation if you can support one. If you cannot, say not consolidated and when you will review.
  • Next care: imaging, specialist, physiotherapy — as facts, not a negotiation with the employer.

If this is a follow-up attestation, change the function and the dates. An identical form every two weeks with 'toujours en arrêt' and no exam is how the file gets a designated physician.

Worked example (fictional)

Factory shoulder — first attestation

Fictional 34-year-old. Do not copy into a real CNESST form.

Date de l'événement: 20 août 2026. Manutention répétitive de bacs à 160 cm, douleur brutale épaule D sans chute.
Diagnostic: tendinopathie supra-épineuse D, sans déficit neurologique. Liée au travail selon l'histoire et l'examen d'aujourd'hui.
Examen: arc douloureux 80–120°, Jobe positif, force 4/5 abduction, pas de perte de pouls, cervical libre.
Limitations: pas de travail au-dessus de l'épaule, pas de charge >5 kg du côté D, pas de répétition bras en élévation. Assignation temporaire possible aux tâches au niveau du bureau/contrôle si l'employeur en a.
Consolidation: non. Réévaluation 10 septembre 2026. Arrêt si aucune assignation disponible.
Plan: AINS, physiothérapie, pas d'imagerie aujourd'hui. Prochaine visite ici dans 2 semaines.

  • No functional limitations, only 'arrêt de travail.'
  • Consolidation dated in the past while you are still ordering investigations.
  • English-only free text on a French form so the file cannot be coded.

No. Same idea — first medical report after a work injury — different statute, different terms, different submission path. Complete the CNESST form in front of you.

Can I write the attestation in English?

The official physician forms are French. Clinical notes in English may be attached if the form allows, but headings and coded fields should match the PDF. When in doubt, complete the printed fields as labelled.

Who decides temporary assignment?

You state what is medically possible. The employer proposes an assignment. CNESST administers the claim. Do not invent a job that does not exist, and do not refuse all work if a real assignment fits the limitations.
