Manitoba EIA Disability Medical Report

Manitoba Employment and Income Assistance uses a medical report when a client is asking to be treated as a person with a disability for EIA. The program is not asking for a diagnosis trophy list. It is asking whether the impairment is prolonged and whether it substantially limits daily living or work capacity.

What EIA is deciding

EIA disability category is a provincial income-assistance decision. It is not AISH, not SAID, not PWD, and not CPP Disability. Download the current medical form from the Manitoba government, not a photocopied booklet a worker left in 2018.

The client's worker may have already collected the social history. Your job is medical: conditions, treatment, function at home and (if asked) toward employment, and how long this has lasted or will last. If the person is applying because they cannot sustain competitive work, say so with examples. If they function independently at home and the only issue is job market, do not dress that up as a medical disability.

You are not the EIA adjudicator

You certify clinical facts. The department decides category and benefits. Fees for form completion are a clinic policy issue unless a current EIA process pays them.

Who completes the medical report

A physician or nurse practitioner who knows the file, or a specialist for the dominant condition. If you met the person today for the first time, say that and attach what you verified. A walk-in that copies the client's handwritten list without an exam is a weak report.

How to complete the clinical sections

  • Conditions you can support, with onset years.
  • Current treatment and whether the person adheres to it. Uncontrolled because they cannot afford the drug is different from refusing treatment.
  • Daily living: personal care, meals, meds, community, social communication. Frequency of help.
  • Work capacity if the form asks: hours, attendance, need for extra breaks, why a usual job would fail.
  • Duration: already 12 months, expected 12 months, or likely permanent. Do not write permanent for an untreated major depression you just started medicating.
  • Prognosis and review date.

Worked example (fictional)

Chronic pain and depression — EIA medical

Fictional 55-year-old. Do not copy into a real EIA report.

Conditions: chronic widespread pain (fibromyalgia, 2018); major depressive disorder, recurrent, current episode moderate (index 2016, this episode 2025). Followed here since 2022.
Function: showers with a bath bench, needs 45 minutes and a rest after. Partner prepares most meals. Manages pills from a dosette the pharmacy fills. Leaves the house 1–2 times/week for appointments; grocery stores are abandoned after 15 minutes because of pain and panic.
Work: last paid job 2024, cashier, left after repeated absences. Cannot sustain a 4-hour shift without lying down. I do not expect competitive employment in the next 12 months at this severity.
Treatment: duloxetine 60 mg, CBT wait-list, physiotherapy declined (transport). Restriction remains on treatment.
Duration: impairment >12 months; likely ongoing at this level. Review 12 months.

Why EIA asks for more

  • Problem list without function.
  • Duration blank.
  • Contradicting the client's section without saying why.
  • Writing as if this were a federal DTC form.
  • Missing practitioner identifiers.

Frequently asked questions

Is EIA disability the same as CPP Disability?

No. EIA is Manitoba income assistance. CPP Disability is federal. A client may apply to both. Complete the form in front of you.

What if I think the person can work with accommodations?

Write that, with the accommodations and the remaining limits. Do not certify total disability to be 'on their side' if that is not your exam.

Can a nurse practitioner sign?

If the current EIA medical form lists NPs, yes. Stay inside the professions named on that PDF.
