Ontario Accessible Parking Permit (APP)
Ontario's Accessible Parking Permit is issued by ServiceOntario. Part A is the applicant. Part B is a regulated health professional certifying that a listed mobility, lung, heart, or vision criterion is met. There is no fee for the permit. Your job is the medical eligibility, not the sun visor.
What Part B is certifying
Download the current application (SR-LV-129) from Ontario's form repository or a ServiceOntario centre. The eligibility list is statutory-style: walking only with a specified device or another person; FEV1 under one litre; portable oxygen; NYHA Class III or IV; severe walking limit from an arthritic, neurologic, musculoskeletal or orthopaedic condition; specified visual acuity or field; or mobility severely limited by other impairments.
You also choose permanent, subject to change, or temporary (up to 12 months). Permanent permits can later be renewed without re-certification. Do not tick permanent to save the patient a trip if the condition is a fracture or a time-limited post-op limit.
Who may sign Part B
Ontario lists: physicians, nurse practitioners (extended class), physiotherapists, occupational therapists, chiropractors, and chiropodists/podiatrists. Stay on that list. A registered nurse who is not an NP cannot certify.
How to complete Part B
- Confirm identity matches Part A. Signing a blank Part B for the patient to fill later is how the wrong person gets a permit.
- Tick the criterion you can defend. One is enough. Five weak ticks look like you did not read the list.
- Lung and heart criteria need numbers or a class you have. If you do not have an FEV1, do not use the FEV1 box — use the walking criterion if that is what you examined.
- Vision: use an eye report or your measured acuity. Confrontation fields are a poor substitute for the 20-degree rule.
- Duration: temporary, subject to change, or permanent, as the form defines them.
- College and registration number, signature, date. A stamp is not a signature.
You do not collect identity documents or mail the package. The applicant files online, by mail, or in person. Misuse of a permit is municipal enforcement. If you believe the person no longer meets a criterion, that is a clinical conversation, not a call to bylaw.

Heart failure — subject-to-change APP
Fictional 44-year-old. Do not copy into a real SR-LV-129.
Part B. Criterion: cardiovascular disease NYHA Class III (documented by cardiology May 2026; last clinic 2 weeks ago). Walks 50–80 m on the flat then dyspnoea requiring a rest. Not on portable oxygen. FEV1 not the issue. Vision not the issue. Duration: subject to change (5-year permit, re-certify on renewal) — function may improve if CRT-D optimization works. Not certifying permanent today. Not a 12-month temporary post-op. I am the family physician, CPSO # on the form. Patient completes Part A and files with ServiceOntario.
Why ServiceOntario cannot issue
- Part B unsigned, or signed by a profession not on the list.
- No criterion ticked.
- Permanent ticked for a clearly temporary injury.
- Heart or lung boxes ticked with no supporting class or volume anywhere in the file you can stand behind.
- Wrong form (another province's permit, or an old SR number).

Frequently asked questions
Is the permit free?
Yes. Ontario charges nothing to get, renew, or replace an APP. Your clinic may still charge for completing Part B if that is your uninsured-services policy.
Can a physiotherapist sign?
Yes, if they are a registered member of the College of Physiotherapists of Ontario and the criterion is within what they assessed. The current Ontario page lists the colleges.
Temporary vs subject to change vs permanent?
Temporary: up to 12 months, re-certify to renew. Subject to change: 5 years, re-certify to renew. Permanent: 5 years, renewal without re-certification. Pick the one that matches the condition, not the one that is least paperwork.
