OSAP disability verification
OSAP disability verification is the medical form a student uses so Ontario student aid can treat them as a student with a disability — including the Canada Student Grant for Students with Disabilities and related OSAP disability supports. It is not ODSP, not T2201, and not a statement that they cannot study. It asks whether a disability is permanent, or persistent or prolonged, and how it affects the student’s studies.
What the verification form is asking
The student completes their OSAP application. You complete the disability verification. OSAP uses this to determine disability-related grant eligibility, reduced course-load rules, and related supports. “Permanent disability” and “persistent or prolonged disability” are program definitions on the form and on ontario.ca — not a synonym for ODSP or for CRA’s marked-restriction test.
Download the current Disability Verification Form from the student’s OSAP account or the official Ontario student-aid pages. The form is revised. Do not reuse a 2019 PDF from a campus accessibility office shared drive if the student has been issued a new one.
You are not the financial-aid office
You certify diagnosis, duration, and functional impact on studying. OSAP decides funding. Fees are billed to the student unless your clinic policy says otherwise. Do not promise a grant because you signed.
Who may complete it
- A regulated health professional listed on the current form — typically a physician, nurse practitioner, psychologist, or other professional named in that year’s instructions.
- Campus accessibility staff do not complete the medical verification unless they are also a listed professional acting in that role.
- Stay inside the condition you treat. A sports-medicine physician should not verify a primary psychotic disorder they have never assessed.
- If you only met the student once for “OSAP paperwork,” say how long you have known them and what records you reviewed.
How to complete the clinical sections
Write the impact on studying: reading, lectures, exams, attendance, group work, internships, stamina. A diagnosis of ADHD without functional examples is why these forms come back. Duration matters: OSAP distinguishes permanent from persistent or prolonged. Read the boxes on this year’s form instead of assuming last year’s wording.
- Diagnosis and year of onset, as supported by the chart.
- Whether the impairment is expected to be permanent, or persistent/prolonged for the period the form defines.
- Functional limits in an academic setting, with or without usual treatment.
- Recommended academic accommodations you can actually support (extra time, reduced load) versus supports that belong to the accessibility office.
- Medications and side effects only when they affect studying (sedation, frequent appointments).
- Do not copy an ODSP Health Status Report or a T2201 into this form. The questions are different.
Worked example (fictional)
Persistent disability — university student, family practice
Fictional 22-year-old second-year student. Do not copy into a real OSAP form.
Condition: ulcerative colitis, diagnosed 2021, two steroid-requiring flares in the last 18 months, currently on biologics. Duration: persistent/prolonged; not a one-term injury. Expected to continue through this academic year and beyond. Academic function: during flares (weeks at a time) cannot sit through 3-hour exams or attend consecutive labs because of urgency, fatigue, and infusion days. Baseline between flares: reduced stamina, unpredictable absences. Treatment: infliximab; gastroenterology shared care. Restriction is residual and episodic despite treatment. I am the family physician since 2020. GI consult 2026-04 attached. This is OSAP verification, not ODSP or T2201.
Why OSAP or the school sends it back
- Diagnosis with no academic function.
- A professional not listed on the current form.
- Permanent ticked for a sprain that will heal this semester.
- Missing student identifiers, unsigned pages, or an outdated PDF.
- Contradicting a full course load with no accommodations while claiming they cannot attend class.
Complete OSAP verification from the visit

Frequently asked questions
Is OSAP disability the same as ODSP?
No. ODSP is income support with its own Health Status Report. OSAP disability verification is for student aid. A student can have one, both, or neither. Complete the form in front of you.
Does a T2201 replace this form?
No. An approved Disability Tax Credit may be useful context, but OSAP still wants its own verification completed by a listed professional on the current form.
Can I recommend a reduced course load?
You can describe why a reduced load is medically appropriate. OSAP’s academic-progress rules for students with a disability are set by the program. You do not set the 40% threshold; you describe function.
