Occupational form

Police recruit medical

A police recruit medical is a fitness-for-hire examination for a police service. Municipal services, provincial services, the RCMP, and the Ontario Association of Chiefs of Police constable-selection process each use their own PDF and often a designated examiner. It is not a generic pre-employment physical, not a fire-recruit medical, and not an MTO driver medical — even though driving and sudden incapacity matter.

What the service is screening for

The job includes shift work, driving, use of force, weapons in some roles, and sudden physical demand. The form asks about vision, hearing, cardiovascular risk, seizures, insulin-treated diabetes, musculoskeletal limits, and conditions that could cause sudden incapacitation. Psychological suitability is often a separate assessment. Do not turn this physical into a psych report you were not asked to write.

Use the service’s current package. If they require a designated occupational-health clinic, do not complete a substitute on family-practice letterhead and call it equivalent.

Designation matters

RCMP and some municipal processes name who may examine. If you are not on that list, stop. Scribeberry can still help a designated examiner fill the official PDF from the exam they just did.

Who completes it

The examiner the recruiting package names — often a physician at an occupational-health vendor, sometimes a designated RCMP examiner. The candidate completes history pages. Do not fill those for them. Do not certify a service you are not designated for.

How to complete the exam form

  • Identity against the recruiting ID, not a nickname.
  • Vision and hearing as the form specifies (corrected acuity, colour, visual fields).
  • BP, heart, lungs, MSK, neuro — including anything that would drop them on a use-of-force encounter.
  • Medications that sedate or impair judgment.
  • Diabetes, seizure, cardiac history with the detail the form asks — not a shrug.
  • Fit / unfit / further information, against this service’s standard, not “seems healthy.”
  • Who receives the result. Recruiting, not the candidate’s Instagram.

If colour vision or acuity fails the printed standard, record the numbers. Do not pass someone because they “really want the job.”

Worked example (fictional)

Municipal police recruit — designated occupational health

Fictional 24-year-old. Do not copy into a real recruit medical.

Service form as provided. Corrected acuity meets the printed standard, colour screen passed, hearing whispered-voice and form audiogram within range.
BP 118/74, no murmur, full MSK, no seizure history, no insulin. Not on sedating meds. Candidate history pages completed by the candidate.
Fit for this municipal recruit medical on today’s exam. Psychological assessment is separate — not done here.
Not RCMP unless this package says RCMP. Not a fire medical.
I am the designated examiner for this vendor.

Why recruiting sends it back

  • Wrong service’s form.
  • Examiner not designated.
  • Vision numbers missing.
  • Candidate history completed in the clinician’s handwriting without attribution.
  • Passing a standard the numbers do not meet.

Draft the police recruit medical from the exam

In Scribeberry, open Forms, search for police recruit medical, or upload the service’s current PDF. Generate from the encounter and your notes, then review every field before you sign. Scribeberry does not pre-map this form. You still review and sign. Scribeberry does not send the file to the police service.

Frequently asked questions

Can my family doctor do this?

Only if the recruiting package allows it. Many services require a designated occupational-health examiner. Read the instructions on the PDF.

Is this the same as a commercial driver medical?

No. Police recruiting has its own vision and sudden-incapacity standards. A CCMTA driver medical does not replace it.

What about mental health history?

Answer what this form asks. A separate psychological assessment is common. Do not hide a condition the form names, and do not volunteer a novel if they did not ask and it is not relevant to sudden incapacity or safety.

Does Scribeberry submit this to the police service?

No. Scribeberry helps you complete the PDF. You or the vendor return it through recruiting. Keep a copy under occupational-health rules.
