WorkplaceNL physician report
WorkplaceNL is the workers’ compensation board for Newfoundland and Labrador. The physician’s report is how the claim learns the diagnosis, the mechanism, and whether the worker can go back. It is not WorkSafeNB, not WSIB Form 8, and not an EI certificate. Use WorkplaceNL’s current physician report.
What WorkplaceNL is asking
First reports open the medical file: what happened at work, what you found, what you diagnosed, treatment, and work capabilities. Progress reports update those facts. Offshore, fishery, and construction files still need usable restrictions — not “off work, rest.”
Use WorkplaceNL’s current form or electronic channel. Do not mail a photocopied Ontario Form 8 to St. John’s. Reporting rules and fees are on WorkplaceNL’s health-care provider material.
Work relatedness is a claim issue
Describe the history the worker gave you and your findings. If the story is inconsistent with the injury pattern, record both. WorkplaceNL adjudicates entitlement.
Who completes it
The physician or nurse practitioner who assessed the work injury. Other professions use the reports WorkplaceNL lists for them. Do not complete a first report for a visit you did not do. If you are covering a colleague’s follow-up, say so.
How to complete the report
- Worker identifiers, employer, date of injury, date of exam, claim number if known.
- Mechanism in the worker’s words, then exam of the injured area.
- Diagnosis that matches findings. Bilateral pain with a unilateral mechanism needs an explanation.
- Treatment today, imaging, referrals.
- Work status with specific limits: lifting, shift length, sea days, driving, cold exposure — whatever the job actually involves.
- Next review. An open-ended off-work note is how the file stalls.
If the worker is going back to a vessel or a remote site, say whether they can do that job safely, not whether they can sit in your waiting room.
Worked example (fictional)
WorkplaceNL — lumbar strain, plant worker
Fictional 44-year-old. Do not copy into a real WorkplaceNL report.
DOI: 2026-08-18. Exam: 2026-08-19. History: worker reports twisting while pulling a tote, sudden left lumbosacral pain. No leg weakness, no saddle anaesthesia, no bowel/bladder change. Exam: lumbar spasm, SLR negative, neuro intact in the legs. Dx: acute lumbar strain. Tx: activity as tolerated, NSAID if suitable, PT, no imaging today. Work: no lifting >10 kg, no repeated bending, no ladders. Can do seated inspection. Reassess 7 days. Not WSIB. Not WorkSafeNB.

No. WorkplaceNL is Newfoundland and Labrador. WorkSafeNB is New Brunswick. Use the board for the workplace that employed the person.
What if the injury happened on a vessel?
Still complete WorkplaceNL’s report if this is their claim. Restrictions should speak to sea duty, not only clinic ambulation. Other federal marine medicals are a separate process.
First report or progress?
First visit for this injury is a first report. Later visits are progress. Do not send a second first report months later unless WorkplaceNL asked for that document.
