WSIB Form 6 — Worker’s Report of Injury/Disease

Form 6 is the worker’s report of injury or disease to WSIB. The worker signs it. You may help a patient fill in dates, employer name, and what happened, but you do not certify Form 6 as the health professional — that is Form 8. WSIB now steers workers to report online; the paper PDF is no longer the channel WSIB advertises.

What Form 6 is — and is not

Form 6 is the worker’s story of the injury or illness: when, where, what they were doing, who they told, and whether they lost time. It is not a diagnosis. It is not work restrictions. It is not your clinical note. If the patient needs a medical report, that is Form 8 (first report), Form 26 (progress), or a Functional Abilities Form.

WSIB’s Form 6 page states the PDF worker’s report is no longer available and directs workers to report online. If someone still arrives with an old paper Form 6, do not treat it as the current filing method. Point them to WSIB’s online services and to what WSIB currently asks workers to have ready.

Do not sign as the worker

If you help type, you are a scribe for their report. They review every line. They sign — on paper if a paper form is still in play, or they submit the online report in their own account. Signing Form 6 with your billing number is how a file gets two contradictory “worker” statements.

Who signs what

Worker (or a person they authorize) completes and submits Form 6 / the online worker report.
You complete Form 8 if you treated the work injury — your report, your signature.
The employer has Form 7. Do not fill Form 7 for the employer unless you are that employer’s clinician and the form in front of you is actually Form 7.
A parent or substitute decision-maker may help a minor or an incapable adult. That is their authority, not yours.

How a clinician can usefully help

Date of injury as the worker reports it, not the date you first charted it unless those are the same.
Employer legal name if they only remember the job-site nickname — they still have to confirm it.
A plain description of what happened in their words. Do not upgrade “I felt a pop lifting” into a radiology report.
Body part they say was hurt. If your exam is a different region, that goes on Form 8, not as a rewrite of their story.
Whether they lost time, told a supervisor, and saw a clinic or ER.
Claim number if they already have one. Do not invent one.

If the worker cannot complete the online report in your office, give them WSIB’s current reporting page and complete your Form 8 on time. A missing Form 6 is a worker/employer process issue. A missing Form 8 is yours.

Worked example (fictional)

Helping a worker describe a transfer injury — not signing Form 6

Fictional 29-year-old personal support worker. Do not copy into a real worker report.

Worker’s words (they sign / they submit):
DOI: 2026-08-25, 14:10, long-term care home, employer as on their pay stub.
What happened: catching a sliding resident during a bed-to-chair transfer; sudden right low-back pain. Told charge nurse the same shift. Left early. Saw family physician next day.
Lost time: yes, remainder of shift and next day.
I did not sign Form 6. I completed Form 8 from my exam the next morning (lumbar strain, no red flags, modified duties).

Why Form 6 files stall

Clinician signed the worker section.
Paper Form 6 submitted after WSIB stopped offering that PDF.
Injury date that disagrees with Form 7 and Form 8 with no explanation.
Using Form 6 as a progress note or a restrictions form.
A diagnosis written in by the clinic that the worker never reviewed.

Help with Form 6 language, then complete your own form

Can I complete Form 6 for my patient?

You can help them get the facts onto the form or into WSIB’s online report. They must review and submit as the worker. You do not sign Form 6 in place of them.

Is Form 6 the same as Form 8?

No. Form 6 is the worker’s report. Form 8 is the health professional’s first report. WSIB wants both. One does not replace the other.

WSIB says the Form 6 PDF is gone. What do I do in clinic?

Point the worker to WSIB’s current online reporting. Complete Form 8 yourself. Do not invent a replacement PDF and call it Form 6.
