Transport Canada Aviation Medical
A Canadian aviation medical is completed by a Civil Aviation Medical Examiner (CAME) for Transport Canada. Category 1, 2, 3, and 4 medicals are not interchangeable. If you are not a CAME, do not complete the Medical Examination Report as the examiner. If you are a CAME, Scribeberry can help fill the official PDF from the exam — you still review and sign.
CAME only
Transport Canada appoints CAMEs. A family doctor who flies on weekends is not a CAME by virtue of a PPL. Regional aviation medical officers adjudicate problem files; you do not 'just stamp Category 1' because the applicant has a check ride on Friday.
Use the current Medical Examination Report and the medical standards in the Canadian Aviation Regulations / Handbook for Civil Aviation Medical Examiners. Applicant history on their part of the form has to match what you find. Discrepancies go in your comments, not in the shredder.
This is not a provincial driver medical
A RoadSafetyBC DMER or SAAQ driver report does not issue a pilot medical. A marine certificate does not either. Complete the aviation form if you are the CAME for that applicant.
What the exam has to cover
- Licence category requested. Category 1 (professional) is a different bar than Category 3 or 4.
- Vision, including colour vision where the category requires it, and correction as worn for flying.
- Hearing as the standard specifies.
- Cardiovascular, neurologic (seizure, migraine with aura, TIA), psychiatric, metabolic (especially insulin), and substance use.
- Medications that impair, including recently started ones the applicant forgot to list.
- Your recommendation: fit, fit with limitation, or defer to Transport Canada / RAMO. Defer is a legitimate outcome.
ECG, bloodwork, and specialist reports follow the age and category schedule, not your clinic's 'annual bloods' habit. If the standard requires an ECG this year, do it. If it does not, do not invent a delay.
Worked example (fictional)
CAME — Category 3, migraine history
Fictional 31-year-old. Do not copy onto a real MER.
CAME exam 28 Aug 2026. Category 3, PPL, not commercial. History: migraine without aura, 3–4/year, last 5 months ago, no neurologic deficit, no opiate, treated with ibuprofen. Not flying with headache. No seizure, no insulin, no untreated apnea. Alcohol within limits declared. Exam: visual acuity meets Category 3 with correction, colour Ishihara pass, BP 122/76, heart normal, neuro normal. No ECG required this cycle for this age/category on the current schedule. Applicant form omitted the migraines; added in examiner comments after interview. Recommendation: fit Category 3. Advice: no flying during or for 24 h after a migraine. Not a Category 1 assessment. If I were not a CAME I would not sign this MER.
Why Transport Canada comes back to the CAME
- Wrong category.
- Incomplete vision or colour testing.
- Applicant history that does not match your exam, with no comment.
- Insulin, seizure, or recent psychosis issued fit without RAMO involvement.
- Signed by someone who is not a CAME.
