RAMQ Online Access Request (SELAT)
SELAT is the clinician's request for RAMQ services en ligne — telematic access to bill, consult insured-service files, and use the professional portals. It is not a patient form. It is an access-control file on you: identity, permit, place of practice, and the services you are asking RAMQ to open.
What you are actually requesting
In Quebec, many RAMQ professional tasks run through services en ligne rather than paper. Before you can bill insured services, look up a patient's eligibility, or use certain professional téléservices, RAMQ has to grant you (or your clinic's authorized users) access. The request package is often searched as SELAT — demande d'accès aux services en ligne / téléservices.
Use the current French form from ramq.gouv.qc.ca. English pages, if they exist, lag. Field labels will be in French: nom, prénom, numéro de permis, lieu de pratique, type d'accès demandé. Complete them in the language the form requires. Do not translate official field names into English on the PDF.
Scribeberry cannot grant RAMQ access
This is an identity and authorization request to RAMQ. Filling the PDF does not open the portal. RAMQ issues credentials after it validates your permit and practice. Keep the signed request and any RAMQ acknowledgement in your credential file, not in a patient chart.
Who files it
- The professional named on the permit (médecin, infirmière praticienne spécialisée, other billed professions as RAMQ lists them).
- A clinic director or responsible professional (professionnel responsable) when the request is for a lieu de pratique or for staff users under that site.
- New graduates, locums, and clinicians moving from another province who need a Quebec billing number and portal access before the first insured visit.
If you are only joining an existing GMF or clinique as an employee, ask whether the site already has a responsable who adds users. Duplicate SELAT requests for the same permit at the same site are a common delay.
How to complete the request
- Identité: legal name as on the Collège des médecins (or your ordre) permit, date of birth, and numéro de permis. Nicknames fail the match.
- Numéro RAMQ professionnel if you already have one; leave blank only if this is a first registration and the form allows it.
- Lieu de pratique: official clinic name, address, and the establishment number if RAMQ issued one. A PO box or a home address when you work in a hospital will bounce.
- Type d'accès: billing, consultation of eligibility, specific téléservices. Tick only what you need. 'All services' when the form wants a list is how requests sit in a queue.
- Mandat / utilisateur délégué: if a secretary or billing clerk will submit claims under your number, the form usually needs that person named and your signature authorizing them.
- Pièces: copy of the permit, a void cheque or banking form if payment is involved, and any RAMQ annex the current instructions list.
Sign where the form says signature du professionnel. A clinic manager cannot sign your permit access unless the form has a separate responsable block. Date it. If RAMQ still uses a secure portal upload rather than mail, follow that year's instructions on the official site.
Worked example (fictional)
Locum family physician — first Quebec portal access
Fictional clinician. Do not copy into a real RAMQ request.
Professionnel: Dr. A. Bouchard, permis CMQ 012345, first RAMQ billing number pending. Lieu de pratique: GMF Les Érables, 4400 rue Ontario E, Montréal. Site already bills RAMQ; responsable de clinique is Dr. C. Nguyen. Accès demandé: services en ligne — facturation des services médicaux, consultation de l'admissibilité, téléservices professionnels linked to that site only. Utilisateur délégué: billing clerk M. Patel, to submit claims under this permit once RAMQ confirms. Annexes: copy of CMQ permit, signed authorization for the délégué, clinic letter confirming the locum start date of 8 Sep 2026. Note: not requesting establishment-level admin rights. Not a patient file. Keep in the clinician credential folder.
Why RAMQ sends the request back
- Name or permit number that does not match the ordre.
- Lieu de pratique that RAMQ does not have on file.
- English free-text in fields that must match French legal names.
- Asking for admin rights for a whole établissement when you are a locum.
- Missing signature of the professionnel or of the clinique's responsable when both are required.

Use the official RAMQ form and complete labels as printed. RAMQ's professional forms are French. Do not rename fields in English on the PDF.
I already bill RAMQ on paper. Do I still need this?
If you need services en ligne — eligibility checks, electronic billing, or other téléservices — RAMQ still has to grant access. Paper billing in the past does not automatically open the portal.
