SGI Saskatchewan Medical Report
Saskatchewan Government Insurance uses medical reports for injury claims after a crash and, separately, for medical fitness to drive. Read the PDF. An injury-benefit form is not a driver medical, and a driver medical is not your opinion on who caused the collision.
Pick the right SGI form
Injury claims want diagnosis, function, work or usual-activity status, and a plan. Driver medicals want licence class, vision, cognition, and sudden-incapacity risk. Completing the wrong one delays both files.
Use SGI's current form. Clinic letterhead that says 'not at fault, still sore' does not help an injury adjuster and does not meet a driver-fitness standard.
Fault is not a medical field
Do not write who caused the crash. Write mechanism and injuries. SGI's liability unit does not need your reconstruction.
Who completes it
Physician or nurse practitioner for most medical reports. Optometrists or others only if that SGI form names them. A walk-in can complete an injury report from today's exam; say you have no prior chart if that is true.
How to complete an injury report
- Claim number and date of loss.
- Occupant role, restraints, ED or not.
- Injuries with exam. Delayed onset is fine if you say it was delayed.
- Function: work, farm duties, driving, sleep.
- Pre-existing same-region problems, labelled as such.
- Treatment and next review. If you expect recovery in two weeks, write two weeks — not an open claim.
If this is instead a driver medical, ignore the injury list and do licence class, vision, seizure/syncope/sleep, cognition, and medications. SGI is applying driver fitness rules, not paying physiotherapy.
Worked example (fictional)
Rural MVC — delayed neck pain
Fictional 62-year-old. Do not copy into a real SGI report.
Claim on file. DOL 10 Aug 2026. Belted passenger, highway rear-end, no airbags, no ED. First visit 14 Aug (delayed neck and interscapular pain next morning). Exam: midline C-spine not tender, rotation 60° each way, no arm pain, no red flags. No head injury symptoms. Work: grain farm, still doing light chores, cannot lift 20 kg seed bags or look over the right shoulder on the tractor for a full day. Pre-existing: OA knees, unrelated. Plan: analgesia, PT in town weekly, review 28 Aug. Expected substantial recovery 4–6 weeks if course typical. Not a driver-fitness exam. I am the family physician.
Why SGI sends it back
- Injury form used as a driver medical, or the reverse.
- No date of loss.
- Fault commentary instead of findings.
- Work or farm status left blank when that is the whole claim.
- Progress note that never changes.

What if SGI asked for a driver medical and the patient wants an injury note?
They are different documents. Do both if both were requested. Do not merge them.
Can I comment on who was at fault?
No. Mechanism and injuries only. Fault is not a clinical certification.
