ICBC Physiotherapy Progress Report

ICBC uses physiotherapy progress reports to decide whether treatment is still changing function after a crash. The adjuster is not asking for a diary of modalities. They want what has changed since the last report, what is still in the way of usual activity, and whether more visits have a job to do.

What this report is for

After a motor vehicle crash in B.C., a claimant may be approved for a block of physiotherapy. The progress report is how you account for that block. Use ICBC's current form or portal fields. Clinic letterhead with 'doing well, continue PT' is not a substitute.

Write to the crash, not to a generic neck template. If this is a rear-end collision with delayed cervical and headache onset, say that. If the main limit is now sleep and desk sitting rather than rotation, say that. The file already has the mechanism. It needs the current bottleneck.

This is not a WorkSafeBC report

ICBC and WorkSafeBC can both be in play after a crash at work. Complete the form in front of you. Do not paste a WorkSafeBC work-status paragraph into an ICBC progress report unless the question is actually about return to work.

Who completes it

The treating physiotherapist. If care transferred mid-block, the current PT completes the progress report from their own exam plus the chart, and says when they took over. Massage, chiropractic, and counselling have their own ICBC reporting. Do not sign those.

How to complete the clinical fields

  • Claim number, date of loss, and dates of this reporting period.
  • Visits attended vs approved. If the claimant missed sessions, say so without editorializing.
  • Current symptoms in functional terms: sitting tolerance, driving, sleep, lifting a child, turning to shoulder-check.
  • Objective change since the last report — same tests, new numbers.
  • What you did in this block that is different from home exercise. If the answer is 'the same ultrasound,' the next block is hard to justify.
  • Remaining barriers and a proposed next block with a review date, or a recommendation to stop.
  • Other care the claimant is getting (physician, psychology, specialist) so ICBC does not think you are the whole file.

If function has not moved in four to six weeks, write plateau and what you think should happen next. A progress report that looks identical to the last one, with another 12 visits requested, is how files get questioned.

Worked example (fictional)

Cervical strain after rear-end crash — week 8

Fictional 29-year-old. Do not copy into a real ICBC report.

Claim: ICBC. DOL: 2 Jun 2026. Reporting period: 30 Jun–28 Jul 2026. Attended 8 of 8 visits.
Crash: belted driver, rear-ended at a light, airbags not deployed. Delayed neck pain and occipital headache next morning. No LOC, no radicular symptoms.
Then (week 4): rotation 50° L/R, sitting 20 min, avoided highway driving, headache 4 evenings/week.
Now: rotation 70° L, 65° R, sitting 45 min at a desk with breaks, highway 30 min with increased awareness, headache 1 evening/week after long screens. Shoulder-check still guarded on the left.
Exam: no neuro deficit, Spurling negative, deep neck flexor endurance 18 s (was 8 s).
Barriers: prolonged laptop flexion, leftover kinesiophobia on left shoulder-check.
Plan: 4 further visits over 3 weeks focused on graded driving exposure and desk setup, then discharge to independent program unless new deficit. Not requesting imaging. Family physician aware; no other rehab this period.

Why ICBC asks for another report

  • No numbers compared with the last report.
  • Requesting more visits without a new goal.
  • Ignoring the claimant's actual limits (driving, childcare, desk work) in favour of a pain score only.
  • Mixing in pre-existing neck complaints without saying what changed after the crash.
  • Wrong claim number or a date of loss that does not match the file.

How often does ICBC want a progress report?

Follow the schedule on the approval or the form in front of you. If the portal asks at the end of a treatment block, that is the report. Do not wait until the claimant asks for more visits with nothing on file.

What if the claimant also has a WorkSafeBC claim?

Complete each insurer's form separately. Same injury can generate two files. Do not assume one report is filed with the other.

Can I recommend more treatment if I think it will help?

Yes, if you can name the remaining barrier and what the next block will change. If you cannot, discharge and say so.
