SAAQ Medical Report
The Société de l'assurance automobile du Québec (SAAQ) asks for a medical report in two common situations: fitness to hold a driver's licence, and clinical information after a crash that SAAQ insures. The PDF in front of you tells you which. Do not complete a driver-fitness form as if it were an injury claim, or the reverse.
Which SAAQ report you are holding
Driver medicals are about licence class, vision, cognition, and conditions that cause sudden incapacity. Injury reports after a collision are about diagnosis, function, and recovery for SAAQ's compensation side. Use the form SAAQ sent or published. Clinic letterhead is not a substitute.
Field names are French: classe de permis, acuité visuelle, troubles de la conscience, limitations fonctionnelles. Keep them. SAAQ adjudicates against its own standards, not against a B.C. DMER you remember from residency.
Licence fitness is not a CNESST file
A crash at work can involve SAAQ and CNESST. Complete each agency's form. Do not paste consolidation dates into a driver-fitness report, and do not put licence-class opinions on an attestation médicale.
Who completes it
Usually a physician; some SAAQ forms accept other professionals for named sections (vision, for example). Stay inside the professions listed on that PDF. If SAAQ asked the driver for a specialist report (neurology, sleep, psychiatry), a family-medicine summary is not that report.
How to complete a driver-fitness report
- Classe de permis. A class 1 truck is not a class 5 automobile.
- Vision as the form specifies. Do not tick 'adequate' if you did not measure.
- Neurologic events: last seizure, syncope, TIA, with dates. 'History of seizure' without a last-event date is not usable.
- Sleep apnea: diagnosis, treatment, and whether the person uses it. Untreated moderate–severe apnea matters more than the label.
- Diabetes with hypoglycemia risk, especially if insulin and a commercial class.
- Cognition and psychiatric illness that affect judgment or impulse, with functional driving examples if you have them.
- Medications that sedate. Name dose and timing relative to driving.
If you are instead completing a post-crash injury report, switch frames: mechanism, injuries, current function, expected recovery, and other insurers already on the file. The licence-class boxes can stay blank if that form does not ask them.
Worked example (fictional)
Class 1 — treated sleep apnea
Fictional 58-year-old. Do not copy into a real SAAQ report.
Permis: classe 1. Motif: renouvellement / condition connue. Vision: 6/6 correction, champs confrontatifs pleins. Sommeil: SAOS sévère diagnostiqué 2023, IAH 38 → 4 sous CPAP. Conformité machine 6.8 h/nuit, 95 % des nuits (rapport 90 jours). Pas de somnolence diurne, Epworth 5. Pas d'endormissement au volant déclaré. Neuro: pas de syncope, pas de crise. Glycémie: non diabétique. Médicaments: aucun sédatif. Alcohol: 3 bières/semaine, pas en quart. Opinion: condition traitée et documentée. Pas de contre-indication médicale à la classe 1 sur cet examen. SAAQ décide. Joindre le rapport de conformité CPAP.
Why SAAQ cannot close the file
- Wrong form (injury claim vs driver fitness).
- Missing licence class.
- Sleep, seizure, or syncope mentioned with no dates and no treatment.
- English headings written over the French PDF.
- A specialist opinion requested and a family-medicine paragraph sent instead.

You can decline a third-party form if it is outside your knowledge. If you have the care relationship and the facts, complete what you can and mark what you cannot. Licence consequences belong to SAAQ.
