WorkSafeBC Physiotherapy Initial Report
The WorkSafeBC physiotherapy initial report is the first clinical snapshot after a work injury. Claims staff use it to confirm the injury, see what the worker can still do, and decide whether the treatment plan matches the job. It is not a SOAP note pasted into a PDF.
What WorkSafeBC wants on day one
Contracted musculoskeletal providers usually file this through WorkSafeBC online services. Paper or PDF versions still show up in walk-in clinics, locum coverage, and out-of-province files. Use the current report, not a scan from a shared drive.
The report has to tie three things together: the mechanism of injury, the objective findings today, and a work status that a case manager can act on. 'Low back pain, treat as needed' does not do that. 'Lumbar strain after a lift from a pallet at 40 cm; flexion to mid-shin; no red flags; no lifting over 10 kg for 10 days' does.
Check the claim before you treat
WorkSafeBC expects you to confirm claim status before starting a course of care. If the claim is not accepted yet, say so on the report and still document what you found. Billing follows the fee schedule and your contract, not this page.
Who completes it
- The treating physiotherapist who did the initial assessment.
- A physiotherapy student may collect findings; the registered PT signs.
- Do not have the front desk invent range-of-motion numbers from the referral.
If you are a contracted concussion or vestibular provider, use that stream's report, not the generic musculoskeletal initial. Home and neurological physiotherapy has its own non-contracted invoicing path.
How to complete the clinical fields
- Claim number, date of injury, and date of first physiotherapy visit. These three must match the worker's story and the physician report if you have it.
- Diagnosis in functional language. Name the structure and the mechanism. Leave ICD theatre for the physician if you were not the diagnostician.
- Objective findings: range, strength, special tests, gait, and what the worker could not do in the clinic today.
- Work status: full duties, modified duties with a weight/time/posture limit, or off work. Name the barrier in job terms (ladder, overhead reach, sitting 45 minutes).
- Treatment plan and expected number of visits. A plan without an end date reads like an open account.
- Red flags you screened and the result. Negative is useful. Blank looks like you skipped it.
If the worker has already seen a physician, do not contradict Form 8/11 unless your exam does. If it does, write the discrepancy and the finding, not a debate.
Worked example (fictional)
Warehouse lumbar strain — initial PT
Fictional 38-year-old. Do not copy into a real WorkSafeBC report.
Claim: accepted. DOI: 12 Aug 2026. First PT visit: 15 Aug 2026. Mechanism: two-person lift of a 24 kg carton from a pallet at mid-shin; partner stepped away; sudden lumbar catch, no radiation, no saddle, no bowel/bladder change. Exam: flexion fingertips to mid-shin, extension 50% with end-range ache, SLR negative, no motor/sensory loss, no clonus. Walks independently. Palpable guarding L4–L5 right. Work: order picker, repetitive lift 5–20 kg from floor to 150 cm, 8-hour shifts. Currently off work. Cannot stoop or lift from floor without pain 7/10. Plan: education, graded lumbar mobility, hip hinge practice, core endurance. Twice weekly, 4 weeks, then review. Expected modified duties (no floor lifts, 10 kg limit, waist to chest only) by week 3 if recovery typical. No imaging indicated today. Advised to keep walking. Follow-up 18 Aug.

Confirm claim status first. If you assess before acceptance, document findings and say the claim status is pending. Payment follows WorkSafeBC's rules and your contract, not this guide.
