WorkSafeBC Form 11 — Physician’s Progress Report
Form 11 is the physician’s progress report to WorkSafeBC. Clinics usually hold a combined Form 8/11 pad or PDF: Form 8 is the first report, Form 11 is later visits. Progress still needs a work status and what changed — not a reprint of the original mechanism. It is not a physiotherapy report and not a WCB-Alberta progress form.
Form 8 versus Form 11
WorkSafeBC publishes a combined Physician’s Report (Form 8/11). Send Form 8 within three business days of the first visit if you think the worker may be off beyond the day of injury. Form 11 is how you update that file. If you are on a follow-up, you are in Form 11 territory even if the paper still says 8/11 at the top.
Use WorkSafeBC’s current file. Check claim status before you assume payment. Progress visits are not a chance to rewrite the accident history unless new facts appeared.

Who completes Form 11
The physician still treating the WorkSafeBC claim, in good standing with the College of Physicians and Surgeons of B.C. Nurse practitioners use the report types WorkSafeBC lists for them. Physiotherapists have separate forms. Do not sign Form 11 for a visit you did not do.
How to complete a progress visit
- Claim number, date of injury, date of this exam.
- Current diagnosis if unchanged — say unchanged. If changed, say what is new.
- Findings compared with last visit. Range, strength, neuro, wound — whatever matches the injury.
- Treatment this interval and whether the worker attended.
- Work: full duties, modified with specific limits, or off work, and a review date.
- Medications that affect safety (opioids, sedating muscle relaxants) if the job is driving, heights, or machines.
- Referrals outstanding. A specialist wait is not a reason to leave restrictions blank.
If function has plateaued, write plateau and what you want next (imaging with a reason, specialist, job-site mediation, discharge). Another identical Form 11 with “continue off work” and no plan is a stall, not a report.
Worked example (fictional)
Form 11 — shoulder strain, week 4
Fictional 52-year-old carpenter. Do not copy into a real Form 11.
Claim accepted. DOI 2026-07-28. This visit 2026-08-25. Injury: right rotator-cuff strain after overhead fastening. First report already sent (Form 8). Now: active abduction 120° (was 90°), night pain improved, no neuro deficit, no red-flag fever or deformity. Tx: PT 6 visits, naproxen. Attended 5 of 6. Work: no overhead, no ladder, can do waist-height framing and paperwork. Not full duties. Reassess 2 weeks. Not a physio report. Not ICBC CL19.
Why WorkSafeBC questions a Form 11
- No work status on a progress visit.
- Mechanism paragraph copied from Form 8 with no new exam.
- Claim not checked, or a claim number that is not this worker.
- Signing a physiotherapy or ICBC form and calling it Form 11.
- Off-work notes with no review date.

Is Form 11 a separate PDF from Form 8?
WorkSafeBC’s usual physician report is the combined Form 8/11. Form 8 is first visit; Form 11 is progress. Use the current combined file and complete the progress sections on follow-up.
When do I send Form 11?
On later visits while the claim is open and you are still treating. Follow WorkSafeBC’s reporting rules and fee schedule — not clinic memory of “every two weeks.”
Can I use Form 11 for an ICBC crash?
No. ICBC has its own medical reports (including CL19). WorkSafeBC Form 11 is for a WorkSafeBC claim.
